Referrals

Do you know someone that needs home health care?

At Amistad Homecare, our patients come first, so we appreciate your referrals. Please read more about us if you are interested to learn more about our unique agency. Below, we have additional information about our Referral process and our Face-to-Face Encounters. Downloadable forms are also linked below so you can download them at your convenience. Amistad Homecare's referral process accepts referrals from:

  • Physicians
  • Hospitals
  • Skilled Nursing & Rehab facilities
  • Community Service Programs
  • Friends & Family


In order to initiate the referral process, all we need is:

  • Patient's name, address & phone number
  • Patient's insurance information
  • Physician's name, phone number & fax
  • Any pertinent medical information such as a History and Physical (H&P) and specific orders, if any
  • Signed admission order by the certifying physician
  • Signed and completed Face-to-Face Encounter Form (which we have available to download on the right)

Face-to-Face Encounters

Due to the Affordable Care Act, a face-to-face encounter must occur within the 90 days prior to the start of care, or within the 30 days after the start of care. Documentation of such an encounter must be present on certifications for patients which starts of care on or after January 1, 2011. 

The physician must include the following information about the face-to-face encounter:

  • Date of the encounter
  • Indication that the encounter was related to the primary reason for home health care
  • An explanation of why the clinical findings of the encounter


For physicians who work in collaboration with a nurse practitioner or a clinical nurse specialist, or who supervise a physician's assistant, the face-to-face encounter ma be carried out by that non-physician practitioner who must document the clinical findings. 

Only a physician, however, may:

  • Order home health services
  • Certify that a face-to-face encounter has occurred
  • Certify that other eligibility criteria are met, such as medical necessity and home bound status 
  • The Face-to-Face Encounter Must
  • Be made by the physician responsible for certifying home health (or non-physician practitioner - nurse practitioner or clinical nurse specialist in consultation with the physician, or a physician's assistant supervised by the physician).
  • Be related to the primary reason for which the patient requires home health services.
  • Be made within 90 days prior to, or within 30 days of the start of care.
